Verifying your vaccination with UCLA

UCLA faculty, staff, students and others who will be living, working and learning on campus or other UCLA properties during the fall must verify their vaccination status at least 14 days prior to the start of the term. (Employees who work in UCLA hospitals and clinics must submit verification by Sept. 1.)

  • Enrolled students 
    Students, including student employees, can submit proof of their vaccination by logging into the Arthur Ashe Student Health & Wellness Center’s electronic patient portal and clicking on the Medical Clearance section. More information on the verification process and a link to the portal are available on the Ashe Center website.

    Students will be able to privately check their status using the COVID-19 Clearance Portal, which can now also be accessed through the UCLA Mobile app, available on the App Store and the Google Play Store.
  • Employees
    Staff, faculty and non-student employees can confirm their vaccinations through the UCLA COVID-19 Symptom Monitoring System in one of several ways: (1) give UCLA consent to check UCLA Health records and the California vaccination database to verify their vaccination; (2) upload an image of their CDC vaccination card (or non–U.S. equivalent); or (3) upload an official medical record from their health care provider.

    Employees should complete the symptom monitoring portion of the survey even if they are not planning to come to campus on the day they upload their vaccination information. Similarly, employees who will not be coming to campus during the fall term should still upload their information and complete the survey so that they will not be flagged for noncompliance with the policy.

    While employees will not receive immediate confirmation of their vaccination clearance status through the Symptom Monitoring System, they will be able to privately check their status using the COVID-19 Clearance Portal (UCLA logon ID required); supervisors will also be able to use the portal to check the status of employees who report directly to them.

    Note: The UCLA Mobile app, which is available on the App Store and the Google Play Store, now includes access to the Symptom Monitoring System and the Clearance Portal.
  • UCLA Health employees
    UCLA Health personnel have been provided information on how to upload their vaccination proof, which will be reviewed by UCLA Occupational Health Services. UCLA Health has also launched a clearance dashboard for managers to view employees’ vaccination status, exceptions and COVID-19 surveillance testing results.
  • Non-matriculating students
    UCLA Extension students, visiting students and those in executive education programs will receive guidance on how to verify their vaccination status through their program department.
  • K–12 students
    Students age 12 and above who are eligible for vaccinations will receive information from the UCLA Lab School or the Geffen Academy at UCLA about notifying their program of their vaccination status.


Exception requests

While UCLA strongly urges all those who are eligible to get vaccinated, in certain very limited circumstances, students and employees may be granted a medical exemption, an accommodation based on religious beliefs or disabilities, or a deferral based on pregnancy. To request an exemption:
