The University of California has issued a systemwide COVID-19 vaccination policy that requires all UC students, faculty, staff, academic appointees, postdoctoral researchers and trainees, as well as anyone accessing UC facilities or in-person UC programs, to be fully vaccinated prior to the fall term — with limited medical exemptions and accommodations based on disabilities or religious beliefs, as well as deferrals for those who are pregnant.
An individual is considered fully vaccinated when 14 days have passed since they received a single-dose vaccine or the second dose of a two-dose vaccine. UCLA faculty, staff and students living, working and learning on campus or other UCLA properties during the fall term should have been vaccinated by Sept. 9. If you have not yet been vaccinated, you should do so as quickly as possible.

Those who have chosen not to be immunized and have not received an approved exception (or a deferral based on pregnancy) do not meet UC’s health and safety condition for physical access to UC facilities or in-person attendance at UC programs or events and may be subject to discipline, up to and including dismissal from employment.

Upload your vaccination information
UCLA faculty, staff, students and others living, working and learning on campus or other UCLA properties during the fall must verify their vaccination status.
- Enrolled students
Students, including student employees, can submit proof of their vaccination by logging into the Arthur Ashe Student Health & Wellness Center’s electronic patient portal and clicking on the Medical Clearance section. More information on the verification process and a link to the portal are available on the Ashe Center website.
Students will be able to privately check their status using the COVID-19 Clearance Portal, which can now also be accessed through the UCLA Mobile app, available on the App Store and the Google Play Store. - Employees
Staff, faculty and non-student employees can confirm their vaccinations through the UCLA COVID-19 Symptom Monitoring System in one of several ways: (1) give UCLA consent to check UCLA Health records and the California vaccination database to verify their vaccination; (2) upload an image of their CDC vaccination card (or non–U.S. equivalent); or (3) upload an official medical record from their health care provider.
Employees should complete the symptom monitoring portion of the survey immediately after verifying their vaccination information, even if they are not planning to come to campus on that day. Similarly, employees who will not be coming to campus during the fall term should still upload their information and complete the survey so that they will not be flagged for noncompliance with the policy.
Employees will be able to privately check their status using the COVID-19 Clearance Portal (UCLA logon ID required); supervisors will also be able to use the portal to check the status of employees who report directly to them.
Note: The UCLA Mobile app, which is available on the App Store and the Google Play Store, now includes access to the Symptom Monitoring System and the Clearance Portal.
- UCLA Health employees
UCLA Health personnel have been provided information on how to upload their vaccination proof, which will be reviewed by UCLA Occupational Health Services. UCLA Health has also launched a clearance dashboard for managers to view employees’ vaccination status, exceptions and COVID-19 surveillance testing results. - Non-matriculating students
UCLA Extension students, visiting students and those in executive education programs will receive guidance on how to verify their vaccination status through their program department. - K–12 students
Students age 12 and above who are eligible for vaccinations will receive information from the UCLA Lab School or the Geffen Academy at UCLA about notifying their program of their vaccination status.
Exception requests
While UCLA strongly urges all those who are eligible to get vaccinated, in certain very limited circumstances, students and employees may be granted a medical exemption, an accommodation based on religious beliefs or disabilities, or a deferral based on pregnancy. To request an exemption:
- Students
Students, including student employees, may submit an exception request through the UCLA Immunization Requirements page. - Employees
Non-UCLA Health employees should use the UCLA COVID-19 Symptom Monitoring System to submit a request. Please visit the UCLA Insurance and Risk Management’s How to Request a COVID-19 Vaccine Exception page for details and guidance before initiating the process.
Additional information on exemptions, accommodations and deferrals is available on IRM’s COVID-19 Related Accommodation or Workplace Adjustment Information page. Exception requests should be made no later than Sept. 9. - UCLA Health employees
